How to Submit an SSA Application for a Name Change in Arizona

The first step of changing your name on your car registration in AZ is performing an official name change with the Social Security Administration. This condition must be satisfied, as the MVD requires a revised Social Security card prior to updating your records. To request a name change and obtain a corrected Social Security document, applicants will be first required to present the mandatory paperwork. Such documents include proof of U.S. citizenship (if applicable), name change, age and identity. When performing an Arizona DMV name change, drivers can provide their U.S. birth certificate or passport to prove their citizenship. As proof of age and identity, you can provide your current driver’s license, state-issued identification card or valid passport.
