Arizona Name Change on Driving Licenses

Drivers who have undergone a legal name change in Arizona due to marriage, divorce or court order must to perform a drivers license name change. Drivers who successfully apply for a name change must report the case within 10 days. Failure to adhere to the MVD-imposed regulations is punishable by law. Prior to starting their driving license name change in AZ, motorists are obliged to submit a name change request with the Social Security Administration (SSA). To complete this procedure, applicants may make an in-person visit to a SSA office or a mail-in inquiry to finalize their name change.

The Arizona Social Security Administration Name Changing Procedure

Before requesting an Arizona drivers license name change with the Motor Vehicle Division, licensees will need to correct their name with the Social Security Administration. A mandatory part of the DMV name change process is verifying the information with the SSA. Motorists who need to request a name change in AZ in such case, will be obliged to satisfy specific conditions. The first step to obtain a corrected Social Security card is presenting the required documentation to the SSA. To finalize your process to change names, you will be obliged to provide the following paperwork:

  • Proof of legal name change
  • Proof of U.S. citizenship (if applicable)
  • Proof of age and identity

The SSA will accept your U.S. birth certificate or U.S. passport as proof of citizenship. Applicants are allowed to provide their valid passport, state driving license or Identification Card as proof of their age and identity. Finally, to prove their Arizona official name change,  motorists will need to provide one of the following documents:

  • Marriage certificate
  • Divorce decree
  • Certificate of Naturalization displaying the new name
  • Court paperwork authorizing the name change

Note: The Social Security Administration does not accept photocopies or notarized copies. Additionally, the presented paperwork cannot be expired.

To finish the process prior to changing your name on your drivers license in AZ, you will need to fill out an Application for a Social Security card. Licensees must then present or mail the application form along with the aforementioned documents to a nearby SSA location. Once their inquiry has been processed, applicants will receive the updated card showing their new name. Note that your Social Security Number will remain the same.

Changing Your Name on Driving Licenses With the Arizona MVD

Licensees applying for a driving license name change in Arizona must address the state Motor Vehicle Division to finish the procedure. Upon changing your personal information with the SSA, you will be asked to allow two days for a system update. After that, motorists will be able to submit a name change request. Currently, Arizona drivers license name changes can be completed either in person or via mail. An in-person change of name on driving licenses in AZ will require you to visit a local MVD branch or an authorized third party office. Once there, you will need to present documentation verifying both your new and prior names. Such proof of identification will include your marriage certificate or license, divorce decree, naturalization/citizenship certificate or court order.

Changing your name on your drivers license in Arizona may be also completed via mail. A mail-in DMV name change may require drivers to submit the applicable identification documents verifying their new name. After the Motor Vehicle Division processes their name change inquiry, licensees have the option to request a duplicate driving license showing their new name. Note that to obtain drivers license replacements in Arizona, motorists may need to pay an MVD-imposed fee.
